Attention: Octogenarians (80-89 years old)
What : RELEASE OF SECOND QUARTER 2024 CASH BENEFIT
When : July 30, 2024 (Tuesday)
Time : 8:00 to 11:00 in the morning (no extension)
Where : Old Clubhouse Lobby
Requirements:
1. If claiming in person, bring your OSCA Muntinlupa ID plus photocopy
2. If you cannot come in person, a representative can claim it for you provided you comply with the following requirements:
– Authorization letter duly signed by the senior beneficiary
– Original and photocopy of OSCA ID with 3 original signatures of the senior
– Original and photocopy of the representative’s government-issued ID
– in lieu of a print out of the picture of the senior beneficiary together with thre representative holding a 2024 calendar of the current month, present a picture shot from the cellphone of the representative and show to the OSCA staff.
3. For a deceased person claimant:
-Death certificate of the deceased
-OSCA ID of the deceased
-Original ID of the claimant’s representative
