DISTRIBUTION of BENEFITS for OCTOGENARIANS
Dear AHV Community Members,
We would like to inform you that personnel from the Office of the Senior Citizens Affairs (OSCA) will be distributing the allowance for those seniors belonging to ages 80yrs old to 89 yrs old (octogenarians).
Distribution will take place at our Multi-Purpose Pavilion on Thursday, 10th of February, from 8:30am to 11:30am.
Kindly bring the following:
1. Valid OSCA Seniors ID
2. Photocopy of the Seniors ID
For those representatives who will claim the allowance on behalf of the octogenarians, the following must be brought & presented:
1. Authorization letter
2. Photocopy of the Seniors ID with 3 signatures
3. Photocopy of the ID of the representative
4. Picture of the octogenarian & representative taken together which is stamped with 2021 calendar.
For registration of Octogenarians (aged 80-89 yrs old), forms are available at the pay-out site.
You may submit your completely filled out forms & requirements tomorrow during the payout activity by the OSCA representatives.
