NATIONAL ID Registration

Dear AHV Community Members,

We are pleased to inform you that the Philippine Statistics Authority (PSA) personnel will accept & process registrations for National ID in our village from Feb 14 to Feb 19, at the Multi- Purpose Pavilion, from 9am to 4pm.

This is open to all AHV members, residents, tenants, household staff & village workers.

Please note that only 200 registrants are allowed per day, on a first-come first-served basis; number stubs will be issued as early as 8:30am per day.

All registrants must bring the completely filled-out form & present 1 valid ID.

A. For registrants 5 to 17yrs old
– Birth Certificate (original copy) or
– School ID showing date of birth & address of the cardholder

B. For 18yrs old & above
-any government-issued ID

Forms are available at the administration & security offices.

National ID cards will be delivered to the registrant’s home address via registered mail.

Everyone shall be subject to temperature scanning and must fill-out health declaration forms upon entry to the venue.

Further, the Association will deploy guards & marshals to strictly enforce health protocols such as wearing of face mask & social distancing.
