Non-Resident must be endorsed by an AHVA resident, village association, or company-employer.

Print & fill out the form in block letters
STEP 3:
Take note of our sticker fees and method of payment
Commercial Vehicles:
- 4 wheels - P 4,000 per vehicle
- 6+ wheels- P 6,000 per vehicle
Replacement sticker:
(For broken or damaged windshield)
- Resident - P 150 per sticker
- Non-Resident - P 300 per sticker
- For new cars (transfer from old vehicle with valid sticker)
- Payment of full fee of sticker

- Cash on your booking day
STEP 4:
Book your appointment
Owners of multiple vehicles need only to book one (1) appointment for all your vehicles. If, however, you have more vehicles than we can process in 15 minutes, we will work with you during your appointment to set up additional appointment days and times for your other vehicles.
