AHVA Bulletin No. 2024-1104
Dear AHV Neighbors,
For those who were not able to claim their rice allocation and food gift boxes last week, please be advised that you have until Friday, November 22, 5:00pm to claim it at the AHVA administration office.
The City of Muntinlupa has authorized the Association to distribute the said gifts until Friday this week during office hours. Unclaimed rice allocations & gift boxes will be returned and picked up by the City on Saturday morning.
Requirements in claiming the Rice Allocation:
1. Present your Seniors ID
2. If claiming thru a representative, a letter of authorization and Seniors ID of the resident must be brought & presented
Requirements in claiming the gift box:
1. Fill out the Household Profile Data Sheet
2. If claiming thru a representative, the completely filled out data sheet must be brought and submitted to the admin office
Kindly note that this is a 1 HOUSEHOLD, 1 FOOD BOX policy.
As per the City’s directive, these Food Boxes will only be released to those who would be willing to submit their Household Data Profile. Please refer to the attached form following this bulletin.
